Case Study
Mr. Peters, 80 years
old, was discharged from the hospital following a stroke. His primary
caregiver is his 75 year old wife who has hypertension and frequent
episodes of cardiac fibrillation. Their daughter is concerned about
their ability to stay in their home and is limited in what she can do
since she lives 100 miles away. She comes a couple times a week and
helps when she can.
The physician has ordered nursing, physical therapy, occupational therapy,
speech therapy and a home health aide.
Two weeks after Mr. Peters was admitted to home care it became evident
by all the team members that the wife was unable to adequately care
for him and a social worker needed to be ordered for consultation.
