In Module Two, "Planning the Home Visit" you learned about some of the required rules and regulations, information on patient's rights,and financial issues dealing with Medicare, Medicaid, and insurance, for reimbursement of home care. Leadership roles and responsibilities of the care manager to help you support your agency and act in the best interest of your patient's were also addressed.

In "Making the Home Visit" you will apply everything covered in the first two modules in the patient's environment, their home. Topics addressed in this module include more leadership skills as a team manager, and supervisor. Paraprofessional assignments, patient assessment, patient education, patient safety, universal precautions and required documentation in the home care environment are addressed. The information provided in these modules are mainly the highlights of each area. Further readings, education and training, policies and procedures from your own agency and preceptor should be used to augment the topics discussed in these modules.
